Welfare Medical Expense Subsidy System
Gifu City offers medical expense subsidies at hospitals and/or pharmacies to lessen the financial burden on children, those with severe physical/mental disabilities, single parent households, etc.
Things that health insurance does not cover, such as treatment or health exams not covered by insurance, vaccines, document fees, room and meal costs when admitted to a hospital, etc. cannot be covered with this subsidy.

Child medical expense subsidy
This is a system that subsidizes insurance-covered medical treatment costs paid at the hospital and/or pharmacy counter when a child gets injured or sick and receives treatment or medicine, using documents that can confirm the insured person's eligibility for health insurance (such as a Health Insurance Card, Individual Number Insurance Card, or Eligibility Confirmation Document).
Those eligible
- Those with an address in Gifu City
- Those enrolled in health insurance
- Until the end of the fiscal year (March 31st) in which the child turns 18.
*There are no income restrictions
Those ineligible
- Those receiving public assistance
- Those receiving the medical expense subsidy for those with a severe physical/mental disability
- Those receiving the medical subsidy for single parent households
What the subsidy covers
Copays for medical expenses at hospitals and/or pharmacies covered by insurance
Things to be aware of
- Childbirths and New Residents
You are eligible for medical expense subsidies from the date of birth or moving in, provided you submit your application within 30 days of that date. If you apply more than 30 days after the date of birth or the date of moving in, medical expense subsidies will begin from the first day of the application month. - If being examined within Gifu Prefecture
Please present one of the following: your Health Insurance Card, Individual Number Insurance Card, or Eligibility Confirmation Document, along with your Medical Care Expense Recipient Certificate at the medical institution's reception desk. - If being examined outside of Gifu Prefecture (hospitals and/or pharmacies)
If you bring an itemized receipt from the hospital showing which fees should be covered by your insurance to Medical Welfare Division (Gifu City Hall), those fees will be reimbursed to you in a bank account that you designate during the month after you apply for the reimbursement. - If medical treatment becomes expensive
If you're using your Individual Number Insurance Card, when you go to the hospital and/or pharmacy, please show them your welfare medical certificate and also agree to let them access your My Number Insurance Card's payment limit information.
If you are enrolled in Late-Stage Elderly Medical Care Insurance and use an Eligibility Confirmation Document, please present your Medical Care Expense Recipient Certificate and the Eligibility Confirmation Document that states your self-pay limit (limit category) at the hospital and/or pharmacy reception desk.
If you are using a document other than those listed above to verify your health insurance eligibility (health insurance card or eligibility confirmation document), please present it along with your Medical Care Expense Recipient Certificate and your Limit Application Certificate at the hospital and/or pharmacy reception desk.

If your health insurance is applied to these costs, you can apply to receive a reimbursement (the amount depends on your insurance plan, and you can contact your insurance provider for further instructions how to do so).

Single parent household, etc. medical expense subsidy
This subsidy applies to patient copays for medical treatment at hospitals and/or pharmacies received by those in a single parent household as well as children without parents.
Those eligible
- Those with an address in Gifu City
- Those enrolled in health insurance
- The parent and children in single parent households or children without parents
*From the day the application is accepted until April 1 of the fiscal year immediately after the one that your child turns 18 in.

What the subsidy covers
Copays for medical expenses at hospitals and/or pharmacies covered by insurance
Things to be aware of
- If you apply within 30 days of becoming a single-parent household (including cases where there is no one to raise the child), the medical expenses you pay at hospitals and/or pharmacies will be reduced from the date you are recognized as a single-parent household. If you apply after 30 days, the reduction will start from the beginning of the application month.
- To those planning on applying for the child-rearing allowance
After applying for the child-rearing support allowance and medical welfare subsidy recipient certificate at the Children's Support Division, please go to the Medical Welfare Division to apply for medical welfare recipient certificates. - If being examined within Gifu Prefecture (hospitals and/or pharmacies)
Please present one of the following: your Health Insurance Card, Individual Number Insurance Card, or Eligibility Confirmation Document, along with your Medical Care Expense Recipient Certificate at the medical institution's reception desk. - If being examined outside of Gifu Prefecture (hospitals and/or pharmacies)

(You will need these for the guardian and all dependents in the household. For further details, please contact the Medical Welfare Division.) - You may also need a residence certificate, certificate issued by a minsei i’in (social worker), certificate issued by a government entity, etc.

Medical expense subsidy for those with a severe mental or physical disability
This subsidy is applied toward medical expenses at hospitals and/or pharmacies for those with a disability.
List of those eligible
Those with a severe mental or physical disability (Income restriction : Applicable*)
- *Those moving in from outside Gifu City will need an income and tax certificate (所得課税証明書)

Those with a mental disorder
Those with a level 1-2 mental disorder certificate

Wounded/sick retired soldiers
Wounded/sick retired soldiers (conditions apply) that have a level 4 disability certificate
Things necessary to apply
- Please present one of the following: your Health Insurance Card, Individual Number Insurance Card, or Eligibility Confirmation Document
- Wounded/sick retired soldier certificate
- Physical disability certificate
- Document to confirm notifying person’s identity (residence card, driver’s license, etc.)
Those bedridden for six months or longer
Those 65 or older that are bedridden and need assistance eating, walking, bathing, etc.

- Bedridden for six months or longer certificate

What the subsidy covers

- If you apply within 30 days of the date your handbook was issued, the reduction in medical expenses at hospitals and/or pharmacies will apply from the beginning of the month in which the handbook was issued. If you apply after 30 days, it will apply from the beginning of the month of application.
- If being examined within Gifu Prefecture (hospitals and/or pharmacies)
