Welfare Medical Expense Subsidy System
Gifu City offers medical expense subsidies to lessen the burden on children, those with severe physical/mental disabilities, single parent households, etc.
Things that health insurance does not cover, such as treatment or health exams not covered by insurance, vaccines, document fees, room and meal costs when admitted to a hospital, etc. cannot be covered with this subsidy.

Child medical expense subsidy
This subsidy is applies when a child gets injured or sick and uses their health insurance card to receive treatment or medicine.
Those eligible
- Those with an address in Gifu City
- Those enrolled in health insurance
- Children until they graduate junior high school (until the first March 31 after turning 15)
*There are no income restrictions
Those ineligible
- Those receiving public assistance
- Those receiving the medical expense subsidy for those with a severe physical/mental disability
- Those receiving the medical subsidy for single parent households
What the subsidy covers
Copays for medical expenses covered by insurance
Children are eligible to receive this subsidy starting from the 1st of the month that they applied.
Things to be aware of
- Childbirths
You can receive the subsidy for medical expenses incurred for a childbirth if you make an application within 30 days after the birth. - If being examined within Gifu Prefecture
Please bring your health insurance card as well as medical welfare recipient certificate (if you have one) - If being examined outside of Gifu Prefecture
If you bring an itemized receipt from the hospital showing which fees should be covered by your insurance to Gifu City Hall, those fees will be reimbursed to you in a bank account that you designate during the month after you apply for the reimbursement. - Hospital admissions
Please receive a high-cost medical expense certificate from your health insurance union and bring it along with your health insurance card when going to the hospital. - When going to a hospital overseas or purchasing a support good (such as a corset)
If your health insurance is applied to these costs, you can apply to receive a reimbursement (the amount depends on your insurance plan, and you can contact your insurance provider for further instructions how to do so).

- Health insurance card
(if you have recently given birth, you can provide the health insurance card of the individual whose plan the baby will be enrolled in) - Document proving the identity of the applicant

- Itemized receipt showing items covered by insurance
- Document proving the identity of the applicant
- Medical welfare recipient certificate
- Document proving the account number of the account that the reimbursement will be sent to
- When going to a hospital overseas or purchasing a support good (such as a corset)

Single parent household, etc. medical expense subsidy
This subsidy applies to patient copays for medical treatment received by those in a single parent household as well as children without parents.
Those eligible
- Those with an address in Gifu City
- Those enrolled in health insurance
- The parent and children in single parent households or children without parents
*From the day the application is accepted until April 1 of the fiscal year immediately after the one that your child turns 18 in.

- To those planning on applying for the child-rearing allowance
After applying for the child-rearing support allowance and medical welfare subsidy recipient certificate at the Children's Support Division, please go to the Medical Welfare Division to apply for medical welfare recipient certificates. - If being examined outside of Gifu Prefecture
If you bring an itemized receipt from the hospital showing which fees should be covered by your insurance to Gifu City Hall, those fees will be reimbursed to you in a bank account that you designate during the month after you apply for the reimbursement. - Hospital admissions

- Health insurance card

- Child-rearing allowance certificate or survivor's pension certificate
※Those that do not have any of the above certificates, please ask the Medical Welfare Division. - For those that have moved to Gifu City: the parent or guardian’s shotoku kazei shomeisho (income and tax certificate)
(You will need these for the guardian and all dependents in the household. For further details, please contact the Medical Welfare Division.) - You may also need a residence certificate, certificate issued by a minsei i’in (social worker), certificate issued by a government entity, etc.

Medical expense subsidy for those with a severe mental or physical disability
This subsidy is applied toward medical expenses for those with a disability.

Wounded/sick retired soldiers (conditions apply) that have a level 4 disability certificate

Those bedridden for six months or longer
Those 65 or older that are bedridden and need assistance eating, walking, bathing, etc.
