*If you are submitting a notification to Gifu City, you only need to submit one copy.
*If the deadline to submit a notification is on a Saturday, Sunday, or public holiday, you will be able to submit it the following day that city hall or any of its related offices are open.

Birth notifications

A birth notification needs to be submitted for all births within Japan

Period

Within 14 days of the baby being born
 (including the day they were born)

Location

  • Where the person registering the birth currently lives (address registered under the resident registration system, address of unofficial residence or location temporarily staying at)
  • Municipal office of where the child was born

Submitted by

Father or mother
(If there are special circumstances, someone else that lives with the person that gave birth, the doctor, midwife, or anyone else that witnessed the birth in this order can register the child's birth)

Things to bring

  • Mother and child health handbook
  • Birth certificate
    (this form is attached to birth notifications, and make sure to have it filled out by a doctor)

Other

Guardians of children with foreign nationalities must apply for their visa with the immigration services agency within 60 days of being born.

Death notifications

This notification is required for every death that occurs in Japan

Period

Within 7 days that the death was known

Location

Where the person who reported the death or the person who passed away lived at (address registered under the resident registration system, address of unofficial residence or location temporarily staying at)

Submitted by

A relative of the deceased person or a non-related person that lived with the person who passed away.

Death certificate
(this is attached to death notifications, and make sure to have it filled out by a doctor)
